Here at Sunflower Family Dental we feel that nobody should have to forgo proper dental care because they do not have insurance.  Our savings plan was created to give patients without insurance access to a more affordable solution for quality care.

Our plan is designed to eliminate traditional monthly fees and deductibles so you only pay out-of-pocket when you need to have dental work done.  We’ve eliminated the hassles and waiting periods to help you maximize the return on your investment.  You simply pay one low annual fee for free cleanings, x-rays, and exams, in addition to discounts on all the other services we offer in-office.

Standard Membership

Comprehensive & Six Month Recall Exam (2 exams per year)
Annual Digital X-Rays (4 Bitewings And 2 Periapical 2 Set Per Year)
Two Preventative Teeth Cleanings (Deep Cleaning Not Included)
Two Fluoride Treatments
One emergency exam per year (if needed)
15% off all other treatments (some exclusions may apply)

Perio Membership

Comprehensive & Six Month Recall Exam (2 exams per year)
Annual Digital X-Rays (4 Bitewings And 2 Periapical 2 Set Per Year)
Perio maintenance cleanings (Deep Cleaning Not Included), 3 per year
Two Fluoride Treatments
One emergency exam per year (if needed)
15% off all other treatments (some exclusions may apply)
Details:
  1. NO deductible
  2. NO annual maximums
  3. NO waiting periods
  4. NO limitations on pre-existing conditions
  5. Your own calendar year
  6. Membership discounts do not apply to products, cosmetic services, implants, or orthodontic treatment.*
Membership:
  1. Membership enrollment fees must be PAID prior to any dental care procedures.
  2. If the participant does not utilize their savings plan benefits, the plan is NON-REFUNDABLE and offers no additional discounts.
  3. Membership is for one year beginning on the enrollment date.
  4. Membership will automatically renew on the anniversary date unless a written request to cancel is received 30 days prior to the renewal.
  5. Payments for additional dental services are the member’s responsibility. Payment is due on the date of service to qualify for the discounts.
  6. Fees for dental services may change at any time.
  7. It is the member’s responsibility to schedule and keep all appointments offered as part of the Dental Membership Plan. Please notify the office at least 48 hours in advance if you must change an appointment in order to avoid missed appointment fees.
